1、醫(yī)學(xué)研究生學(xué)報(bào)2015年1月第28卷第1期JMedPostgra，Vo1．28，No．1，January，2015·61·論著(臨床研究)老年2型糖尿病患者甲狀腺激素水平與胰島素抵抗的關(guān)系欣冰，郭雯，李杰，王亞梅，陳曉倩，上官海燕【摘要】目的血清甲狀腺激素生理范圍內(nèi)的波動(dòng)可能與胰島素的敏感性存在相關(guān)性。本研究探討在甲狀腺功能正常的老年2型糖尿病(type2diabetesmellitus，T2DM)患者中甲狀腺激素生理范圍內(nèi)的變化與胰島素敏感性的關(guān)系。方法選取2013年9月至2014年5月在南京市市級(jí)機(jī)關(guān)醫(yī)院內(nèi)分

2、泌科就診的老年T2DM患者200例及150例正常對(duì)照者分別納入T2DM組和對(duì)照組。所有研究對(duì)象均進(jìn)行口服75g葡萄糖耐量試驗(yàn)，收集血樣本，并對(duì)入選者進(jìn)行問卷調(diào)查、體格檢查及代謝相關(guān)指標(biāo)的測(cè)定。使用化學(xué)發(fā)光法測(cè)定胰島素、促甲狀腺激素(thyroidstimulatinghormone，TSH)、游離三碘甲腺原氨酸(FI’3)和游離甲狀腺素(FT4)。按照FT3水平三分位數(shù)將研究對(duì)象分成3組，比較各組間Matsuda指數(shù)差異及相關(guān)性。結(jié)果老年T2DM組患者HOMA—IR顯著高于對(duì)照組[(5．18±3．22)(2．35

3、±1．82)]，而Matsuda指數(shù)均顯著低于對(duì)照組[(2．79±1．32)vs(5．81±3．11)](P<0．05)；老年T2DM組FT3水平低于對(duì)照組[(4．05±0．27)pmol／L(4．61±0．5)pmol／L)]，差異有統(tǒng)計(jì)學(xué)意義(P<0．05)；老年T2DM組TSH水平高于對(duì)照組[(2．75±1．07)IU／mL(2．28±0．89)IU／mL]，差異有統(tǒng)計(jì)學(xué)意義(P<0．05)；FI3高水平組Matsuda指數(shù)(4．71±2．64)分別高于m中水平組(3．98±2．31)和低水平組(3．89±
