1、［摘要］目的該研宄主要探討了乳汁淤積的原因及護(hù)理對(duì)策。方法將入選的乳汁淤積產(chǎn)婦80例隨機(jī)分為對(duì)照組與護(hù)理組，對(duì)照組采用常規(guī)護(hù)理手法，護(hù)理組采用中藥熏蒸、乳房按摩以及短波理療治療進(jìn)行治療，考察乳房腫脹，疼痛指數(shù)以及有效率。結(jié)果治療后，護(hù)理組乳房腫脹評(píng)分1級(jí)，2級(jí)，3級(jí)分別26例，12例以及2例；VAS評(píng)分不疼痛患者15例，有效率92.5%;對(duì)照組乳房腫脹評(píng)分1級(jí)，2級(jí)，3級(jí)分別14例，19例以及7例；VAS評(píng)分不疼痛患者9例，有效率80%;對(duì)照組與護(hù)理以上有顯著差別，P［關(guān)鍵詞］乳汁淤積；中藥熏蒸；乳房按摩；短波理療［中圖分類號(hào)］R473.7

6、ferencesintheresultsoftheaboveaspectsbetweenthetwogroupswerestatisticallysignificant，P1.2治療方法對(duì)照組：使用傳統(tǒng)護(hù)理手法對(duì)對(duì)照組患者進(jìn)行干預(yù)，主要手段為在產(chǎn)婦哺乳之前先熱敷并進(jìn)行按摩，產(chǎn)婦哺乳后用吸乳器吸空乳房，最后貼理療貼。護(hù)理組：①選用紫花地丁、蒲公英、赤芍、紅花、銀花等做為中藥配方，將該中藥配方煎至300mL，加入300mL溫水放入熏蒸儀中。對(duì)中藥藥液進(jìn)行加熱，產(chǎn)生蒸汽后，將噴頭調(diào)整至離乳房15cm處對(duì)乳房進(jìn)行熏蒸，配合乳房按摩。熏蒸時(shí)間為30m

7、in/次，2次/d，一個(gè)療程為3d［2］。②經(jīng)過(guò)中藥熏蒸以及按摩以后，使用短波治療儀對(duì)護(hù)理組病例進(jìn)行理療。采用鼓型輻射器對(duì)乳房垂直照射，自動(dòng)調(diào)諧。短波照射距離為4cm,頻率為27MHz,1次/d，一次時(shí)間為12min。1.3療效評(píng)定標(biāo)準(zhǔn)①乳房腫脹評(píng)分：將乳房腫脹程度分為3級(jí)，1級(jí)：乳房腫脹程度較輕，觸及乳房后如觸唇；2級(jí)：乳房腫脹程度中等，觸及乳房后如鼻尖；3級(jí)：乳房腫脹程度較重，觸及乳房后如額頭［2］。②VAS評(píng)分：采用視覺(jué)模擬評(píng)分（VAS評(píng)分）對(duì)所有入選病例進(jìn)行疼痛評(píng)分，最高分10分，分?jǐn)?shù)越高表示越疼痛。0分不疼痛；1?3分表示輕度疼痛

8、；4?6分中度疼痛，7?10分劇烈疼痛［4］。①療效判定：顯效：乳房腫脹程度降低，疼痛評(píng)分降低;有效:乳房腫脹程度或疼痛評(píng)分有一個(gè)降低；無(wú)效：乳房腫脹程度或疼痛評(píng)分均沒(méi)有降低。1
